Please note that the form to be used by consultants or other significant contributors listed as key personnel on PHS funded applications has been updated on the Office of Sponsored Projects website.

In most instances, collaborators from other institutions are part of subaward arrangements and conflict of interest compliance is managed through their home institution.    Less commonly, collaborators may be both key personnel and consultants or other significant contributors.

Often, these roles do not meet the definition of an "Investigator" as defined by PHS rules (see below).    If the collaborator does not meet the definition of Investigator as determined by the PI, then please simply provide this information to your OSP Grants Officer in an email.   In the event that they do meet the definition of Investigator (as determined by the PI), we ask you to provide the form on the website prior to submission of the proposal.

To further clarify, in the vast majority of cases, we would not expect Other Significant Contributors to meet the definition of "Investigator" as defined by PHS.   We are simply asking the PI to affirm this.

The form is now two parts.   The first page should be completed at the time of proposal and submitted to OSP.   The second form is only required if there are responses that indicate a financial interest.   In these cases, form 2 should be submitted directly to the COI officer, Henrike Frowein.

Investigator means the project director or principal Investigator and any other person, regardless of title or
position, who is responsible for the design, conduct, or reporting of research funded by the PHS, or proposed for
such funding, which may include, for example, collaborators or consultants.
