I've been using the one on Paul Gorodyansky's site for a few years now and
have had no problems with it. (My only complaint is the placement of the
cyrillic 'v'--on the 'w' and not the 'v'--but that's a minor thing.)

We had similar problems with several Russian phonetic keyboards from
various sources. The one we got to work reliably was from Fingertip
Software.
http://www.fingertipsoft.com/cyrstart/. It is not free, but does have
a 45 day evaluation version.
