I get my cassette labels, along with my cassettes, from Crown Magnetics.  The
labels come in 8.5x11" sheets which I print using my laser printer (12
cassette labels to a sheet).  Couldn't be easier.  I even use the hole in the
middle to make the label for the reverse of the tape (side B).  There is a
Word template for these label sheets (if anyone wants it I can send it to
them).
