(1)  Did you use the command Select All Document rather than simply 
Select All before changing the header/footer settings?  Also, after 
you uncheck different headers/footers, it might help to toggle the 
setting of another checkbox.

(2)  If the above doesn't work, post your question to the web forum 
to get a response from Martin.
