I will like to recommend the following free tools:
On Windows: DivxLand Media Subtitler with Subtiler plug-in
On Apple: World Caption (directly add timecodes and export to QuickTime)

By the way, there are some free tools to create timecoded caption text or
SMIL files. If you need them, please contact me.
