Doug,
Here is my belated reply.

1. What placement test do you use?

Our own on-line tests, using ExTemplate, assessing speaking, listening, writing 
and reading.

2. Is credit awarded for classes that they place out of? If so, how does that
work in your institution?

No credit, just placement.

3. Are students required to take the exam in-person even though the exam is
online? If not, how have you addressed the potential for academic dishonesty?

Students take the tests on-line during the summer from anywhere in the 
world, or at the latest during Orientation week for freshmen. Rice has an honor 
code and students have to agree to the honor code on the first page of the 
placement test.
