I've been using the one on Paul Gorodyansky's site for a few years now and
have had no problems with it. (My only complaint is the placement of the
cyrillic 'v'--on the 'w' and not the 'v'--but that's a minor thing.)

we have used the phonetic Russian Keyboard availabe at

We have used this one successfully on our lab WinXP machines:

For our Macs, we use the provided Russian phonetic keyboard in OSX 10.4.x

Hi Laura,

We had similar problems with several Russian phonetic keyboards from 
various sources. The one we got to work reliably was from Fingertip 
Software.

http://www.fingertipsoft.com/cyrstart/. It is not free, but does have 
a 45 day evaluation version.
