I just revised one on the IALLT copyright site:
http://www.clas.ufl.edu/llc/copyright/COPYLET.htm

This one is primarily meant for audio lab material.

Of necessity there will be a lot of variation anyway--different
institutions have different needs. Also, the permissions publishers are
willing to grant vary.
