I have posed this question, why the menu bar keyboard icon sometimes 
disappears, to the Apple Systems Engineer who serves our campus, and 
to AppleCare.  They have given me several suggestions, none of which 
has provided a permanent solution.  I, too, would love to know what 
causes this problem, which has affected all the versions of OS X that 
we have used:  10.2.3-10.2.6.

I have found that using Disk Utility>First Aid>Repair Permissions 
will increase the time period between incidents.  It may be that 
repairing permissions on a regular basis would prevent the 
disappearing keyboard icon problem, but I have done things in the 
other order- I repair the permissions when the disappearing icon 
alerts me of the need.
