This also happened to me once. The document wasn't very large -- 20-odd pages, with maybe a dozen footnotes. I fiddled around with it for a while to see if I could fix it, but it persisted. Then it occurred to me to close and reopen the file, which I did, and that solved the problem. I was never able to reproduce it. Which is not to say that closing and reopening will solve the problem for you, but you might try it if you haven't already.
